By signing this form I knowingly waive my rights to sue for injury or death resulting from equine activities at, affiliated, or sponsored by Moonraker, Ltd., unless caused by gross negligence or intentional acts.

Moonraker activities include activities related to boarding horses, handling horses, tacking up, leading horses, trailering, jumping, trail riding, schooling in the rings or fields, and all other equine related activities.

I understand that there are risks inherent in equine activities, including (i) the propensity of a horse to behave in dangerous ways which may result in injury; (ii) the inability to predict a horse's reaction to sound, movements, objects, persons, or animals; and (iii) hazards of surface or subsurface conditions such as hard ground, jumps, contact with stationary objects, etc.

This waiver shall remain valid unless expressly revoked by me.

In consideration for being allowed to participate in Moonraker, Ltd events or activities I, on my on behalf and on behalf of my assigns and personal representatives, releases and discharges Moonraker Ltd., its directors, officers, employees and agents from all claims, demands, rights or causes of action, present and future, whether known, anticipated, or unanticipated, and resulting from, or arising out of, or incident to, my participation in Moonraker, Ltd. events or activities.

In accordance with subsection B of section 3.1-796.132 and subsection 2 of section 3.1-796.132 of the Equine Activity Liability Act, I expressly assume the risk of injury or death to me or my minor child due to an act or omission of ordinary negligence by Moonraker, Ltd or its agents, or employees in the care of the horses and premises including but not limited to the facilities, pastures, trails, roads, and fences.
